Collaborative practice agreements
In roughly 25 states, a mid-level clinician's authority to practice and prescribe depends on a document the group has to keep current.
What the obligation looks like
- An agreement per clinician, per state, naming a collaborating or supervising physician
- Board filing where the state requires the agreement on record rather than on file at the practice
- Chart review, meeting and attestation obligations that vary by state
- Renewal, and re-execution whenever the physician of record changes
The failure that stops billing
When the collaborating physician leaves the group, every agreement naming them becomes invalid on the same day. In states where prescriptive authority and the state controlled substance registration hang off the agreement, the mid-level clinician loses the authority to prescribe immediately, and the group usually discovers it weeks later. One departure can take a dozen clinicians out of compliance across several states at once.
How we manage it
We hold the register of agreements, the physician of record for each, the state filing status and the renewal date, and we re-paper the set when a physician departs. This sits in the Full lifecycle subscription tier. Drafting and board filing for a new agreement is quoted per filing.
